How do I work out the answer?

Meshing gears always turn in opposite directions, so the direction flips at every gear. Count the gears: an odd number means the last gear matches the first; an even number means it is reversed.

Does the size of the gears matter?

Not for this puzzle. Gear size changes how fast a gear spins, but the question is only about direction, which depends purely on how many gears are in the chain.

Do I need to read any language to play?

Barely · the gears and rotation arrows are visual, and the two answers are clockwise and counter-clockwise. The labels are translated into all 25 languages as a helper.
